How Do Global Rankings Work?

Title says it all really, I've been curious about the various rankings that give 100 sus when someone beats a floor or a challenge. Is it server specific? Separated by region or level? I'm curious because I see people higher rank than those who are #1 on my ranking list. Would appreciate any answer, this game is awesome.
