There are many achievements in Honkai: Star Rail version 2.6, I will divide the table according to each series. But the numbers will continue from each table so you can check the Table of Content for a specific achievement easily.
Important: This guide will be continuously updated until all achievements are updated with the correct descriptions and images. UPDATE: COMPLETE.

All of the achievements below can be obtained from the new Trailblaze Mission [Banana Outrage: Battles Without Ninja and Humanity].

1) Epic Hunt: Culmination

Mappō: Conflagration. Complete the Epic Hunt. This is also recorded in the Cosmic Ninjutsu Inscriptions

2) Let's Be Banafriends!

Ba? Nana, Babanana!

3) The Belly of an Architect

If you can't handle even this amount of work, how are you gonna do anything else?
※ Become a Dreamweaver Academy student
You can only get one of the three achievements (The Belly of an Architect / High School Musical / Yuppie Dream)

4) High School Musical

Joining the film crew to taste the Twelve Dreamscape lunch box dishes and collecting Steinway Falcon fragments on camera.
※ Become a Charmony Academy student

5) Yuppie Dream

Do a SWOT analysis in the morning, follow up with a PEST analysis after lunch, and initiate a soulless PowerPoint presentation at night.
※ Become a Fortune Academy student

6) Stick and Rope

There are two types of people in this world, the ones who are unreasonable, and the ones who pull things out of thin air

7) Nobody Knows Slumbernana Better Than Me

There are two types of gunners in this world, the ones who know how to fire a gun, and the ones who know how not to fire a gun

Monkeys revere savagery, and they will die savagely as well

10) Ninja's Path: Past Life

Peek into the reality behind the world of ninjas and sweep the cobwebs out from the Dazzling Ninja Hero's memories

11) Fiendling: Realization

Reality is compassionate — this battle is like the Great Yaoshi extending a fragile spider's thread to a fiendling on the brink of death, stopping just short of going too far.
※ In the Moment of Slumbernana, defeat a small number of Slumbernana Monkeys
You can only get one of the three achievements (Fiendling: Realization / Fiendling: Rout / Fiendling: Without a Trace)

12) Fiendling: Rout

Reality is ruthless — this battle is like the wrathful gaze and demonic fire of the Great Naraka God devouring all fiendlings, sweeping away like wind scattering remnants of clouds.
※ In the Moment of Slumbernana, defeat a good number of Slumbernana Monkeys
You can only get one of the three achievements (Fiendling: Realization / Fiendling: Rout / Fiendling: Without a Trace)

13) Fiendling: Without a Trace

Reality is terrifying — this battle is as if the Great Lan personally hurled billions of divine ninja lux arrows at the fiendlings, purging them completely.
※ In the Moment of Slumbernana, defeat a huge number of Slumbernana Monkeys
You can only get one of the three achievements (Fiendling: Realization / Fiendling: Rout / Fiendling: Without a Trace)

Eager for Battle

14) Bananenlightenment Master

Prevent Wonder Forest's Banacademic Office Staff from using "To the Doctor: Banacademic Report" during the fight. Keep in mind that these enemies are only available in the latest Trailblaze Mission and if you missed it, you need to wait until these enemies are added to Memory of Chaos or Pure Fiction.

First of all, get to the second phase of this enemy. You can see the skill 'To the Doctor: Banacademic Report'. This skill will add 'End-of-Semester Dinner' on the action bar. What you need to do is make sure that the End-of-Semester Dinner icon NEVER reaches the top and take action.

You can delay the End-of-Semester Dinner by Weakness Breaking the enemies. Keep using Rappa's Ultimate to break all of them. Do this until the battle ends and you will get the achievement.

15) Un-banaenlighten-able

Ensure all Banacademic Office members are Weakness Broken at the same time during the fight with Wonder Forest's Banacademic Office Staff. Keep in mind that these enemies are only available in the latest Trailblaze Mission and if you missed it, you need to wait until these enemies are added to Memory of Chaos or Pure Fiction.
All you need to do is Weakness Break all 5 enemies before the battle ends.

16) Dazzling: Break!

Inflict Weakness Break 10 time(s) on enemies using Rappa in a single battle. Pretty straight to the point. Remember that when using Ultimate to enter the Sealform state, her Basic ATK can still reduce Toughness even if the enemies don't have Imaginary Weakness.

17) A Step Like a Malicious Spirit in Pursuit

While using Rappa's technique "Ninja Dash: By Leaps and Bounds," avoid hitting any obstacles while sliding in the "Graffiti" mode. This one is really easy and you can use Trial Rappa to get it. Simply use her Technique and slide in a circle for 20 seconds until the Technique duration ends and you will get this achievement.

Universe in a Nutshell

All of the achievements below can be obtained from the new Simulated Universe: Unknowable Domain expansion.

18) No Ill Will

Follow Herta to uncover the truth behind the "Scepter" system. Just continue the Antinomic Waltz Adventure Mission where you need to complete the SU multiple times until you get the Occurrence Spying on Heaven (III). After that, you will get this achievement.

19) Butterfly on the Ripple

Assist the geniuses in eliminating the Unknowable Domain within the Simulated Universe.
※ Break through Polka Kakamond's Knowable Domain.
After you get the previous achievement, you will unlock the Chaotic Ripples challenge. Complete the challenge and you will get this achievement.

20) Last Essays

Obtain the "singular solution" regarding the future of the universe. Once you get the previous achievement where you complete the Chaotic Ripples challenge, talk to the two geniuses and you will get this achievement.

21) The Aeons THEMSELVES

Listen to a conversation that never existed. Play the SU Unknowable Domain until you get the Occurrence Expectation of Aeons to get this achievement.
Note: I got this one in the (05) Someone's Mausoleum at Conundrum Lvl. 3.

22) Imperfect Scholar

In the Simulated Universe: Unknowable Domain, complete all stages of the Mundanite Assembly. You can only unlock the Mundanite Assembly after completing the Adventure Mission Antinomic Waltz. Once you unlock it, complete all the stages there to get this achievement.

23) Join the Glorious Evolution

While completing Simulated Universe: Unknowable Domain, perform at least 5 modification(s) to Scepters. There are 2 ways to modify Scepters:
(1) Use the Workbench of Creation to modify the Scepter using Cosmic Fragment
(2) Defeat bosses and get the same Scepters you have to upgrade them
For the first option, it's very expensive so you need to save Cosmic Fragments. While for the second option, you need to be lucky in getting the same Scepters you already have.

Note: There is a Curio that modifies the Scepter called Lapispeakers' Flakes. I don't know if this one counts toward the achievement (as I got the achievement in the run that did not give this Curio) but the more you know.

24) Iron Sunrise

While completing Simulated Universe: Unknowable Domain, possess more than 20 Component(s). In short, the Components are like Blessings in this SU Unknowable Domain. You can get them by defeating enemies, occurrences, adventure domains, and the sorts. Get 20 Components and this achievement is yours.

25) Learn to Be "Me"

While completing Simulated Universe: Unknowable Domain, complete Component Overwriting more than 30 time(s). Each Workbench of Creation can only overwrite up to 20 times, so you'll need at least 2 Workbench of Creation to get this achievement (of course, if you have enough Cosmic Fragments to pay for all of them).
Note: You don't have to defeat the boss at Plane 4 to get this achievement. I quit after realizing that I would lose against the boss but still got the achievement.

There are 4 planes and this workbench appears on each/before the boss domain, which means you can interact with the workbench 4 times per run.
Tips: 'Blue' Components are cheaper to overwrite so focus on overwriting those rather than the 'purple' or the 'gold' ones.

26) Imperial Alchemist

While completing Simulated Universe: Unknowable Domain, complete Component Synthesis more than 4 time(s) at the Workbench of Creation. Each Workbench of Creation can only synthesize component once! So you'll need at least 4 Workbench of Creation to get this achievement.
Note: You don't have to defeat the boss at Plane 4 to get this achievement. I quit after realizing that I would lose against the boss but still got the achievement.

There are 4 planes and this workbench appears on each/before the boss domain, which means you can interact with the workbench 4 times per run.

27) Break Free From the Quagmire of Probabilities

Clear Simulated Universe: Unknowable Domain at Conundrum Level 6. Straightforward achievement. Good luck!

28) Kardashev Type-III

Acquire a Component of 3-star rarity in Simulated Universe: Unknowable Domain. Complete the first Adventure Mission for this Simulated Universe where Screwllum teaches you the tutorial, and you will get this achievement.

29) Beyond Cognition

Acquire a Scepter of 3-star rarity in Simulated Universe: Unknowable Domain. You can get a 1-star Scepter right from the start. Go through the floors and increase that Scepter to 2-star. Before entering the last boss stage, use the Workbench of Creation to upgrade that 2-star into a 3-star Scepter and you will get this achievement.

30) Time Dilation

In Simulated Universe: Unknowable Domain, deduct extra Cycles for the first time. The first image on the left is the standard Cycle. When the battle exceeds the standard Cycles, it will consume extra Cycles (on the right).

Just make sure you finish the extra Cycle too (the indicator is 'The enemy has gone berserk' at the top) before the battle ends and you will get this achievement.

31) 1:4:9

Discover traces left by the Lord of Silence. Go to the left side of Herta's Office and investigate the shining spot to get this achievement. If it doesn't appear, try completing the SU Unknowable Domains a few time.

The Memories We Share

32) Sunday Night Live

Locate the three "not quite the same" Dream Tickers and take a picture with them. Once you interact with the 3 Dream Tickers on this map (all of them can be spotted with the Dream Ticker icon on the map), go to the concert area in the middle again and interact with them to get this achievement.

33) Welcome to the Classroom of Academic Freedom

Audit and listen to 3 professional classes at Paperfold University. Make sure you complete the Trailblaze Mission first before trying to get this achievement. Go to the classroom south of the university and attend the first lecture.

Go to the dean's office and talk to the Pepeshi student to get another achievement. Then, teleport to another map before going back to the University map.

Do this 2 times to spawn the Lecturers and attend their classes. By the end, you will get a message from March 7th and also this achievement.

34) Adios, Bottle

Help the Film Club crew complete their film. Make sure to complete the Sound Hunt Ninjutsu Inscription Event first or this NPC won't appear here. Once you complete said event, just go to this studio in the university and talk to the Movie Club Member. Follow what he said and you will get this achievement by the end.

35) What Goes Around, Comes Around

Once again encounter the Pepeshi student who sold your signed photo. Go to the classroom at south of the University and talk to the Fortune Academy Lecturer to start the lecture. By the end of the class, you will interact with a Neighboring Pepeshi who asks to take a picture of you.
Note: Make sure to complete the Trailblaze Mission in the university first or the Lecturer won't appear here.

After that, March 7th will message you to say that your picture is being sold on the school forum. Now, go to the dean's office and you can see the Neighboring Pepeshi again. Talk to her and you will get this achievement.

36) That Is One Big Balloon

Look at the huge bird balloon floating above the center of the stage. Investigate these two NPCs near the center of the concert area and you will look at the huge bird balloon on top of the concert area. Then, you will get this achievement.

37) Learned a Lesson...?

Speak with the student being punished with standing in the corridor. Teleport to the space anchor on the west of the university and walk north a little bit until you reach the first door on your left. Interact with the door and listen in to the conversation before a student will get thrown out. Talk to them and you will get this achievement.

38) Origami House Music

Discover the Origami Bird who sings along with the onstage music. First of all, you need to complete all Advanced Trials in the Sound Hunt Ninjutsu Inscription Event to unlock the Hip Hop Rhythms.

Once you unlock the song, go to the center of the concert stage, switch to the Hip Hop Rhythms, then go to the right side of the stage to talk to the Origami Birds. Then, you will get the achievement.

39) Graduation Contact List

Help all classmates who need to write their yearbooks, and be sure to leave your feedback. Talk to all of the NPCs above and there will be an option where we help write the message. Make sure to complete the Sound Hunt Ninjutsu Inscription Event as most of the NPCs are locked behind that event.

40) Draw It Yourself, Pen!

Leave your graffiti on the wall once more. Make sure to complete the Trailblaze Mission at this university first before you can do this again. Go to the area where you left your graffiti before and get near the empty wall again so the option to spray will appear. Choose any graffiti you want and you'll get this achievement.

41) '39

Read all of Harrison's Emotions with Clockwork. You can find Harrison near the space anchor on the east side of the university. Activate Clockwork on him to change his moods. Make sure to read all of them, including the Happy one (his initial emotion).

42) Aeon Bless Paperfold University!

Listen to the conversation between the two passionate students. Make sure to complete the Sound Hunt Ninjutsu Inscription Event first before going to the location above. There, listen to the 2 NPCs and you will get the achievement.

43) Sweet Dreams in the Cradle

Listen to the conversation between Professor Owl and the birds. You can find Professor Owl on the map from the origami bird's icon. However, this achievement is only possible once you collect all 20 origami birds in Penacony Paperfold University College. After collecting all of them, interact with Professor Owl to get rewards then talk to it again to get the option 'What about your apprentices...'

To get that option, I believe you need to collect previous Origami Birds and complete the Dream Song Adventure Mission (cmiiw). I already collected everything and done every mission so it's hard to tell.

44) Fast & Family

Listen to the conversation between the Racing Club Members. Teleport to the space anchor on the south of the university and walk north until you reach the many booths. Go to the 2 NPCs near Prema and listen to their conversation to get this achievement.

45) Color Your Dreams, Free Your Ideals

Help the real dean of Paperfold University. Go to the dean's office in the University and talk to Mr. Arturo. Use a Calm Gear to unlock the Adventure Mission 'Clockwork: Arturo.' Complete the quest and you will get this achievement (it's pretty straightforward).

46) The Goblet of Fire

Read all of Prema's Emotions with Clockwork. Teleport to the space anchor on the south of the university and walk north until you reach the many booths. You can find Prema on the right side of the booths (east). Activate Clockwork on her to change her moods. Make sure to read all of them, including the Angry one (her initial emotion).

47) Student Club Staff Members

Create your own student club in your imagination. Investigate the desk on the south area of the university and you'll have a chance to create your own student club. Choose anything you want and in the end, you will get this achievement.
Note: If the option doesn't appear, make sure to complete the Trailblaze Mission in this area first.

48) Is This Real?

Locate the club booth for Paperfold University's Concert Band Club. Teleport to the space anchor west of the university (Charmony Academy), and head west to the far end booth. Interact with the Performance Department Director and you will get this achievement.
Note: Make sure to complete the Trailblaze Mission in this area first or she won't appear here.

49) Factory Library (Event)

In Sound Hunt Ninjutsu Inscription, collect a total of 3 musical composition inspirations. You can collect the musical composition from the Inspiration Collection Progress.

50) Right, Right, Right, Right, Right, Right, Right (Event)

In Sound Hunt Ninjutsu Inscription, obtain 3 stars in music challenge of Hard difficulty. Straightforward description, just complete the event and you will get this achievement.

51) Remix 10 (Event)

In Sound Hunt Ninjutsu Inscription, unlock the fourth track of the stage. Once you complete all 3 performances and finish the Adventure Mission for this event, go back to the event page and the Musical Composition will be updated.

Follow Montana's navigation and tap on the icon at the top left screen. The fourth track is called Hip Hop Rhythms. You can unlock it by completing all Advanced Trials.

52) Psycho-Pass

Help all those who are lacking emotions in the Dreamscape to fix their dial. I obtained this achievement after completing the Adventure Mission Clockwork: Arturo. From my understanding, you need to complete every Clockwork Adventure Mission (in other maps of Penacony) for this achievement to be unlocked.

53) Musical Rhapsody (Event)

In Sound Hunt Ninjutsu Inscription, name the band you created. Complete the first event mission you get for the event and you will immediately have the chance to name the band and get this achievement.

54) Natsu Matsuri (Event)

In Sound Hunt Ninjutsu Inscription, complete the first performance of the music party with your band. Complete the Inspiration Collection Progress in the Dreamscape Rock (the 3 initial songs) and you can start your first performance.

55) Heaven and Hell (Event)

In Sound Hunt Ninjutsu Inscription, complete the second performance of the music party with your band. Continue collecting inspiration and you will automatically get to start the second performance.

56) Testify (Event)

In Sound Hunt Ninjutsu Inscription, complete the final performance of the music party with your band. Complete every stage of the rhythm game to start the third and final performance.

57) Eerie Tales

Read all of Ligeia's Emotions with Clockwork. Teleport to the space anchor on the east of the university and head north to the end of the corridor to find Ligeia. Activate Clockwork on her to change her moods. Make sure to read all of them, including the Calm one (her initial emotion).
Note: Make sure to complete the Trailblaze Mission in this area first or she won't appear here.

Fathom the Unfathomable

58) The Curtain Never Falls on Balloons

Break all special Boss Stone balloons at Paperfold University. Teleport to any space anchor in the university and head to the middle area where you can see a huge concert venue with lots of Boss Stone balloons. There are 7 special Boss Stone balloons in this area and you can spot them easily because they spin unlike normal balloons. You can see the image above for the locations of all special balloons.

Attack all 7 special balloons and you will get this achievement. Or just attack all balloons in this area if you want.

59) Farewell, Origami Birds!

Help all Origami Birds return to the Great Tree and the Classroom. I obtain this achievement after collecting every Origami Birds in this area. But if you didn't get it, then you may need to collect Origami Birds in other Penacony areas that you haven't collected yet.
